What Is Direct Primary Care?

Most of the health care system is built for volume. We built Valiant for you.

Direct primary care (DPC) is a membership. You pay one annual fee directly to your doctors. No co-pays and no bill for every visit. In return, you get unhurried appointments, a direct way to reach us, and care that's centered on you instead of on billing codes.

We handle everything from colds and chronic conditions to hormone care and prevention, all under one membership. Lab work and prescriptions still run through your insurance, the way they would with any physician.

  • 6 metabolic and functional assessments a year from MetaFitRx For example: VO2 max: how much oxygen your body can use at full effort, a strong measure of heart and lung fitness. Resting metabolic rate (RMR): how many calories your body burns at rest. DEXA scan: a quick, low-dose X-ray that shows how much of your body is muscle, fat, and bone.

Membership covers your access to us and your visits. Lab work and prescriptions go through your insurance, the way they would with any physician (Valiant Premier includes its quarterly SiPhox lab draws). Valiant membership is not health insurance, and we recommend keeping coverage for hospital, specialist, and emergency care.

Valiant works alongside your insurance.

Direct primary care isn't insurance, and it isn't meant to replace it. Keep your coverage for hospital stays, specialists, surgery, and emergencies. Valiant takes care of the everyday part, and the two work well together.

With a high-deductible plan

  • Everyday care at a flat price. With a high-deductible plan, you often pay full price for office visits until you meet your deductible. With Valiant, your primary care visits are covered by your membership instead.
  • Fewer surprise bills. No co-pays and no per-visit bills from us, so you know what primary care costs before the year starts.
  • Catching things early. Unhurried visits and easy access make it easier to deal with problems while they're small, which can help you avoid bigger costs later.
  • Your plan is still there. Labs, prescriptions, specialists, and hospital care still go through your insurance, the way they always have.
For employers

With a self-insured health plan

  • Fewer routine claims. Self-insured companies pay their employees' claims directly. Moving routine primary care to a flat membership can lower the number of claims your plan pays.
  • Fewer urgent care and ER trips. When your team can reach their doctor the same day, many problems that would have meant urgent care or the ER can be handled by us.
  • Less time away from work. House calls and virtual visits mean no commute and no waiting room, so appointments take less time out of the workday.
  • Simple to offer. Your people join the same memberships as everyone else, at the same prices.

A note on HSAs: Starting in 2026, federal rules let some direct primary care memberships sit alongside a health savings account, but only below set monthly fee limits and other conditions. Valiant's membership fees are above those limits, so please talk with your plan administrator or tax advisor about HSA rules before using or contributing to an HSA while you're a member.

Steven E. Bishop, MD

Co-Founder · Internal Medicine

Steven Bishop, MD, is board-certified in Internal Medicine by the American Board of Internal Medicine and a member of the Alpha Omega Alpha Medical Honor Society. He earned his medical degree at the University of Virginia School of Medicine and completed his internal medicine residency at Virginia Commonwealth University Medical Center, where he served as Chief Resident. He then joined the VCU faculty, becoming Associate Professor and Associate Chair for Education in the Department of Internal Medicine.

Rushed appointments and a system built around billing codes led Dr. Bishop from academic medicine to concierge and direct primary care, where he could build an unhurried, trusted partnership with each patient.

His clinical interests include preventive medicine, men's hormonal health, metabolic health, and nutrition, particularly low-carbohydrate and ketogenic approaches to chronic disease. He's also Chief Medical Officer of Commonwealth Sterile Solutions, which gives him a practical view of how care is delivered at scale.

Leroy B. "Bo" Vaughan, Jr., MD

Co-Founder · Infectious Diseases & Internal Medicine

Bo Vaughan, MD, is a board-certified Infectious Diseases and Internal Medicine physician, clinical researcher, and Richmond native. He studied at Washington and Lee University, earned his medical degree at Virginia Commonwealth University, and completed residency at the University of Maryland Medical Center, where he served as Chief Resident in Internal Medicine before specializing in infectious diseases.

Dr. Vaughan spent more than a decade caring for veterans in the Veterans Health Administration in Baltimore and Richmond. During COVID-19 he served as Hospital Epidemiologist at the Hunter Holmes McGuire VA Medical Center and helped design and run one of Virginia's first outpatient monoclonal antibody infusion programs.

As Medical Director of Clinical Research Partners, he has led Phase II, III, and IV clinical trials of new antiviral and infectious disease treatments. He also serves as President of the Virginia Board of Medicine.

What is direct primary care?

Direct primary care (DPC) is a membership model. You pay a flat annual fee directly to your physician, with no co-pays and no per-visit charges. In exchange, you get unlimited access to your doctor, longer appointments, and care that's centered on you rather than on billing codes.

Is Valiant Medicine only for men?

No. Valiant started as a men's practice, and men's health is still a big part of what we do, but we care for women too, and employers can sponsor memberships for their teams. Same doctors, same approach.

What happened to Valiant Men's Health?

We changed our name to Valiant Medicine now that we care for women as well as men. Nothing about your care has changed: same doctors, same approach, same membership. If you have questions about your membership, just email us.

What does my membership cover?

Your annual membership covers all physician access: unlimited visits (house calls are standard, and virtual visits are available too), 60-minute consultations, direct physician contact, care coordination, and everything we offer, including primary care, hormone optimization, cancer screening, and peptide therapy. Lab work and prescriptions go through your insurance, as they would with any physician.

What's included in Valiant Premier?

Valiant Premier is our executive program, at $8,000 a year. It includes everything in the Valiant Health Program, plus a SiPhox Health Ultimate 360 lab panel drawn at home every 3 months, and 6 metabolic and functional assessments a year from MetaFitRx, such as VO2 max, resting metabolic rate (RMR), and DEXA scans. If you're interested, just ask Dr. Bishop or Dr. Vaughan.

Do I still need health insurance?

Yes. DPC covers your primary care needs, but we recommend keeping health insurance for unexpected hospital stays, surgery, or specialist care outside of primary care. Valiant pairs especially well with a high-deductible health plan.

How are labs and prescriptions handled?

Lab work and prescriptions go through your regular insurance, just as they would with any physician. We help coordinate the process and are happy to talk through costs up front. Valiant Premier members' quarterly SiPhox lab draws are included in their membership.

How do house calls and virtual visits work?

House calls are our standard way of seeing you. We come to you throughout the Richmond area, and it's included in your membership with no added fees. Virtual visits use a secure video platform and work well for follow-ups, medication management, lab reviews, and non-urgent concerns. You can usually be seen the same day you reach out.

How do I reach my physician?

Members can reach Dr. Bishop and Dr. Vaughan directly by phone, text, and secure message. You won't go through a call center or an after-hours answering service. For urgent concerns, we typically respond within the hour during normal hours. For an emergency, call 911.

How is Valiant different from a concierge practice?

Traditional concierge practices usually charge a membership fee on top of billing your insurance for visits. Valiant is a direct primary care practice: your membership covers physician access and visits directly, with no insurance billing for those services. Labs and prescriptions still go through your insurance.

Can my company sponsor a membership?

Yes. Companies can sponsor memberships for their team in the Valiant Health Program, and Valiant Premier for leaders and executives. Same programs, same prices, and we can come to your office for visits. Email us and we'll talk through what makes sense for your company.
